Devon and Cornwall Police has referred itself to the watchdog
A 19-year-old driver has died after he crashed his car into a wall in Plymouth.
It happened on St Simons Lane at the junction with Salisbury Road at around 2am on Sunday 21 January. He had previously failed to stop for police.
The driver, who was known locally, was taken to Derriford Hospital where he later died. A 19-year-old female driver sustained serious injuries and was also taken to hospital where she remains.
Devon and Cornwall Police said it was co-operating fully with the watchdog's investigation.
