An investigation into the sinking of a fishing boat, in which the Exmouth-based skipper died, has concluded that the boat was overloaded and safety equipment didn't function.
Fifty one year old Geoff Ingram lost his life when the Sarah Jayne capsized off Berry Head last September.
The Marine Accident Investigation Branch report, published today, says the trawler had 20 tonnes of fish on board when it was designed for 17.
The extra weight on board made it unstable when waves washed into the boat . and the liferaft failed to open because of the position it was stowed in.
The report also found that an automatic emergency beacon failed to activate and none of the crew were wearing lifejackets.
