An initiative being launched in Exeter is aiming to reduce the number of elderly people who're admitted to hospital when they could be cared for at home.
In many cases older patients are kept in because there's no care available in their communities.
Now the city's branch of Age Concern is working with other organisations to put care and support networks in place so that more of those patients can return home.
Its director Martyn Rogers says hospitals aren't the best places for elderly people unless they need round-the-clock medical care - they can pick up infections, suffer falls, or become institutionalised.
