A report has strongly criticised the former management of Devon county council's social services - and given details of how a mistake allowed a known paedophile to continue abusing two young girls.
Back in 2007 a social worker failed to check records clarifying the risk he posed - and decisions made by social services during that time were based on the incomplete information.
It meant the girls weren't given the protection they needed for another five years, despite concerns raised by their school.
Today the county council has apologised and says its procedures and training have been radically revised.
