A revolutionary new approach to the care of people with complex needs is being introduced in the Exeter area - and has just received 1.5 million pounds in Government funding.
It'll see the creation of a team representing several organisations including the NHS, Age UK and local authorities.
A single front-line worker will assess the needs of the person concerned and liaise with all the other agencies, co-ordinating their care.
